The attached application form, the affidavit and the personal data sheet must be completed 20 days before the start date and submitted to the Education Office accompanied by:

* Certificate of studies

* Photocopy of identity card or passport and residence permit

* SOCIAL SECURITY NUMBER

* VAT NUMBER

* COVID-19 SUNDAY INSTALLATION CERTIFICATE (valid)

The contracts or the approval decision must be presented at least 7 working days before the start, together with the medical certificate mentioned in the attachments (immunisation of trainees).

On the first day you will bring a small photo to the Hospital Education Office.

STUDENT ENTRANCE 

1. The entrance of all students will be carried out by the MAIN ENTRANCE of the hospital.
2. During the first day entrance, students are requested to arrive 10 minutes before the start of their training, having with them their certificate of vaccination or covid-19 infection (valid), in hard copy or in a photo on their mobile phone.

The check will be carried out at the main entrance of the hospital and by the person supervising each student (supervisor or other employee of the department).
