INSTRUCTIONS FOR COMPLETING AN APPLICATION - ISSUING A DECLARATION/AUTHORISATION
Before you start filling in the application form, a DECLARATION OF RESPONSIBILITY is required if the applicant is the patient, or an AUTHORIZATION if the applicant is a third person. Log in directly to gov.gr from the corresponding buttons at the bottom of the application. After the issue is completed, copy the verification code (as in the figure below) and paste it into the corresponding field of the application (DECLARATION/authorisation code (gov.gr)) or, after downloading the application file in pdf format, attach it to the REPLACEMENT FILE field. If you have made the DECLARATION-AUTHORISATION at the CECs, use the camera button and after taking a photo, attach the photo file to the file in the REPLY FILE field. Continue filling in the remaining fields.
